We know from the research that trauma most often precedes addiction. We also know that cognitive techniques carry a bulk of the research evidence for safe and effective treatment of most co-occurring mental health disorders. Given these documented outcomes, clinicians are beginning to find new ways to employ cognitive therapy mechanisms that pack a bigger punch than vanilla CBT. Just when you think you had cognitive therapy all figured out – wait for this one!
The relationship between trauma and addiction is becoming increasingly well established in the literature. This is no surprise, as clinicians working “in the trenches” know all too well that people with unresolved trauma and emotional wounds often turn to substances as a way to “numb out,” forget, or otherwise self-medicate to regulate their emotions. Conversely, people with addictive behavior disorders may drive while impaired, gravitate towards toxic relationships, go to dangerous places to get their substance of choice, or engage in many other behaviors that increase their risk of being traumatized and elevate their vulnerability factors for developing PTSD or other trauma and stress related disorders. It is also known that when cooccurrence is involved, this presents clinicians with a more complicated course of treatment and less favorable treatment outcomes than we see when clients present with only one or the other. The good news is, there is hope! While neuroscience has taught us much about this phenomenon in recent years, evidence-based Cognitive treatments are still show to be at least as effective as many “newer” approaches, and in some circles seem to have almost gotten lost in the shuffle.
Recent protocols have attempted to incorporate PE (prolonged exposure) and other cognitive behavioral treatments with substance use disorder treatments. While further research is needed in this area, the mandate for integration of treatments when dealing with this population seems integral to obtain more favorable outcomes for individuals with these complex conceptualizations. The Battle of the Generations, our favorite “versus” game – Boomers, Millennials, Gen X, Gen Z, and even the newest Gen “Alpha” – each comes with common and unique perspectives and beliefs about how the world works (or should work). Oftentimes, these beliefs and identities clash, and if you’ve ever been to Grandma’s or Auntie’s house for Thanksgiving or Christmas dinner, you have undoubtedly seen or felt them clash HARD. Many trauma courses touch on dissociation as a symptom, but very few trainings dive into dissociation as its own phenomenon, exploring its common presentations, various drivers, and targeted interventions. Dissociation is mysterious, and working with it can be intimidating, but it doesn’t have to be with the right training and practice!
Therapists can sometimes feel overwhelmed by the challenges and confusing presentations of clients who experience trauma-related dissociation, ranging from symptoms to disorders. This highly practical workshop will offer in-depth skills to assess and work with trauma-related dissociation. We will explore four types of phenomena that are included under the umbrella of dissociation, their different symptom presentations, and their different treatments. An emphasis will be placed on what has been called “structural dissociation,” that is, dissociation of the sense of self that results in aspects or parts of self that have undue separation and results in ongoing lack of voluntary awareness and control. This phenomenon is found in a range of trauma-related disorders, from complex PTSD to Dissociative Identity Disorder. Participants will learn strategies for assessment, including specific ways to ask about dissociation. Then we will explore the inner world of dissociative aspects of self and their functions, how they differ from normal ego states, why they remain overly separate, and how to promote gradual integration through the use of therapeutic process, a collaborative therapeutic relationship, and a number of specific techniques. Trauma-related conflicts and phobic avoidance, especially the phobia of inner experience (e.g., thoughts, emotions, sensations, memories, wishes, needs), will be addressed, and their treatment will be described as a central part of therapy.

What is countertransference and how does it happen in therapy? Countertransference is viewed in our profession as the therapist’s reaction to the client’s feelings (i.e. transference) which are being projected – usually unconsciously – onto the therapist. It’s important to manage this inevitable process effectively to avoid ethical and legal dangers. The process of transference and countertransference can be used as a tool for better therapy outcomes and personal growth for you as an evolving clinician!
Although ethics codes and laws guide us, all therapists are vulnerable to finding themselves in the complicated space of ethical dilemmas. By using case examples and depth therapy perspectives, this workshop will help you in linking your attachment style and countertransference experiences. By learning your own blindspots and attachment vulnerabilities, you can inform and minimize risks, and keep yourself out of several types of ethical dilemmas.
